Please fill out the forms in advance if possible and bring your insurance cards to the vaccine clinic in the Hall.

You may also fill out the forms in advance and email them back to kypharmacyrx@gmail.com or Fax us at 833-623-2599 or just bring them to the Vaccine Clinic in the Hall.

We will have extra forms available at the Vaccine Clinic. 

If you do not have insurance and need to pay out of pocket the vaccines are priced as follows:

Flu (high dose -65 y/o or older) $ 99.99
Covid-19 - $175.99
Shingles- (50 y/o or older) $229.99 per shot (need 2 shots 2-6 months apart)
Pneumovax 23 (50 y/o or older) $149.99
Prevnar 13 (50 y/o or older) $284.99
RSV (75 y/o or older) $389.99
